"Yes it is true, indoor plumbing as we know it today is relatively new.The term "out house" dates back to Europe some 500 years ago. Fine inns and such would offer his and hers units, small, about three to four feet square by 7 feet high. For obvious reasons they were built about 150 feet away from the main dwellings! Imagine no heat or electric lights in the dead of winter, that was the way it was with an "out house".
Of course the "out house" made it overseas to the new world. Many of us have probably stumbled upon one or two when playing in the woods in old New England towns. So when did modern indoor plumbing make its move? A young architect named Isaiah Rogers designed the Tremont Hotel in Boston with indoor plumbing in 1829, the country was in awe! Of course there were ancient civilizations that reportedly had indoor plumbing- this was new and exciting to Colonial America.
(Thomas Jefferson was known to have devised a system in his dwelling but this was disposal, using chamber pots and servants.)
It was not until the late 19Th to early 20Th century that homes would have complete plumbing systems. That is why some old homes have strange set-ups. Toilets in small closets right near the kitchen- as long as they were inside that was all they wanted! It is true that people put a high priority on bathrooms, the more the better. I know I can speak for all plumbers, We are happy to install and maintain the plumbing IN your house- remember it was not too long ago when the "in" was "out"!
